I am using a debian linux system with an M-Audio pci-card with breakoutbox for 4 * balanced in.
and commissioned a friend to write a webinterface for it:

it can do 2 * stereo or 4 * mono or a combination of this, streaming mp3.
different bitrates and sound levels per stream.
config of server and filename per stream.
can do live stream to server, live stream to local file, or both. (config per stream)

works great!

also check http://streambox.org/ for a knoppix-based linux boot cd with this software.
